Grandparents Visitation Rights?

We haven't seen nor heard from our now 6 year old granddaughter in 4 months. Mom has primary cust. Dad is estranged son. Mom moved and we can't find daughter, altho we know it's still in FL.
I found a form for court on line.
Has anyone else tried for Grandparents Visitation?
