Motion for Contempt

My ex was ordered to pay the taxes, which he did not do. He is also behind in support payments. Can anyone tell me the title of the motion that I need to file. I have looked everywhere. Is is a motion to show cause for contempt? If so is there an official form or do I make it up? This is for Campbell County, KY

Contact the Child support office and keep pushing. Don't give up, hold them accountable for doing their job!
