My husband and I were divorced in Arizona after 35 years. I was a stay at home mother. I moved to Iowa, he is now in Korea. He is now trying to terminate the permanent support he agreed to pay on the grounds that I now can support myself. I work part time for minmum wage. What is the form that I need to file in Maricopa Co to move the case to Iowa?
Where is his permanant home ? If he still maintains a residence in Arizona it's unlikely a change in venue would be granted . You can may be able to register the decreee in Iowa for enforcement .

If your support agreement does not specifically state that it is "non-modifiable" then it is, by default, modifiable.
Working part time for minimum wage does not demonstrate an ability to support yourself. Does he have anything else you think he'll present in support of his motion?
