how do the judges look at mental health

this is the third time my ex and I have gone back to court because he didn't hold up his part of the order. My child has been diagnosised with bipolor and he wont comply with the doctor's orders. I just want to know what I am headed into and the best advice to make sure I get to make the decisions for my child since he doesn't seem to be. Any help, advise, persnal experience would be very helpful to me. Thanks
