Divorce Certificate Form PH-1682

I am filing an uncontested divorce. Neither my wife or I will be using an attorney. I read online that I need to use Form PH-1682 (The Divorce Certificate) from the Tennessee Department of Health. I emailed the Dept of Health to see where to pick this up ... and my email is being forwarded around. I submitted a Petition for Divorce with the court along with the Marital Separation and Property Agreement notorized by both, and I have a Final Decree of Divorce. What is the Divorce Certificate that I need and is there anything else I need to bring in?
